My Care


FHC provides medical services for adults who wish to follow their own health status.


Preventive care includes health services like screenings, check-ups, and counseling/education used to prevent illnesses, disease, and other health problems, or to detect illness at an early stage when treatment is likely to work best. My Care let’s individuals follow their own screening and prevention path.


A start-up visit is important to help map your path. If you have an illness or a significant family history of any disease, we will meet with you to develop your personal plan of care and screening. After establishing your My Care plan you will be able to navigate your own wellness map.


My Care includes lab draws, immunizations, referrals for special testing (mammograms, colonoscopy, etc.) routine blood pressure, weights and other follow-up measures to help you stay on track for health.


My Care is for adults without chronic disease* who want to help manage their routine care. Please check your insurance benefit description for a complete listing of covered tests and procedures. Your plan may not cover every screening test listed or recommended. We have included the costs to you if your insurance does not cover the testing. (Reduced charges are available with proof of income.)

 

 

 

If you do not have any chronic illnesses (such as HIV, high blood pressure, heart disease, diabetes, Hepatitis, etc.), My Care may work well for you. Annual check-ups are important, but an office visit is not always required. 

 


 Blood pressure- You should check your blood pressure
at least once a year. There are many studies that show
even a few points reduction in blood pressure result in
longer life. High blood pressure is the cause of congestive
heart failure and very high blood pressure causes strokes.
 
 
The goal- blood pressure less than 120/80 mm Hg.

 Body mass (body weight)- You should check your weight
at least annually. Your weight to height is the body mass index or BMI.
 
 
A BMI more than 25 is overweight and may have health consequences.

*If you have a chronic disease and are managing your health, we recommend regular visits for monitoring - Prompt Care visits in addition to a minimum number of physician visits to review your status and your individualized health plan.


Healthy lifestyle choices are key to good health and wellness. 

 

Smoking is stupid. It is deadly. It is dirty and stinky. Stop smoking!

Cigarette and e-cigarettes cause premature death. People who smoke lose an average of about 10-15 years of life (varies by studies).

Other drug abuse is very harmful.

Examples- Cocaine causes sudden death from heart stoppage. Methamphetamine may cause irreversible psychosis. Opiod addiction and overdoseage kills thousands annually. FHC recommends Alcohoics Anonymous or Narcotics Anonymous to help you kick the habit and become healthier.
