REGISTRATION
New patients need to complete a patient registration form before their appointment. The Required Forms can be printed and completed prior to the appointment, or patients should plan to arrive 30 minutes early to fill out the forms.

PAYMENT
FHC accepts most insurance plans as well as offering a Sliding Scale Discount Program. In order to apply for the discount, a completed Sliding Fee Application is required along with a photo ID and proof of income for all household members. The form can be printed and completed prior to your visit, or at the clinic before your appointement.
