We provide quality health care services to all people in the community. Fees are based on a sliding scale and we accept patients with or without insurance, and with Medicaid, Medicare, or Ryan White coverage.
FHC accepts most insurance plans. If you have a co-pay, please be prepared to pay at the time of the visit. Please be advised that patients are responsible for verifying if our providers are in-network and the co-pay amount.
We strive to give affordable healthcare to everyone using a Sliding Scale Discount Program that is based on income. In order to apply for the discount, a completed Income Verification Form is required along with a photo ID and proof of income for all household members.
MEDICAID / MEDICARE
We accept Kansas Medicaid and partner with UnitedHealthcare Community Plans, Aetna Better Health of Kansas and Sunflower Health Plan. Our Social Service team can help with questions or enrollment.
FHC accepts payment through Ryan White. It is the patient’s responsibility to track out-of-pocket expenses. If you reach your Payment Cap, contact your Case Manager and we can work with you regarding your coverage.
Sliding Scale Discount Program
In order to apply, patients must bring one of the following:
• Current Federal Income Tax (1040-1040 EZ Form)
• Two most recent paystubs
• Current unemployment determination letter
• Copies of SSC, pension, trust, SSI disability letter
• Child support check
• Current financial aid papers, scholarships, etc.
• Other applicable sources

Good Faith Estimates
FHC is pleased to provide you with an estimated cost for scheduled medical services if you do not have health insurance. This estimate will include the out-of-pocket cost we predict you will owe for your visit. This information will be provided over the phone when you schedule your visit, with a written copy given when you check in.
This estimate will not include additional services recommended by your provider or requested during your appointment.
