In addition to comprehensive medical care, FHC provides important wrap around services for our patients and community.
SOCIAL SERVICES
The social service team at FHC meets with patients to assist with referrals to Community Resources that will ensure basic needs are met. These referrals may include utility, rent, transportation, food or housing assistance. They can also assist with Medicaid and Disability applications.
Not only does the team meet with patients to support individual and family needs, they also support our providers who identify needs for our patients.
LITERACY
FHC collaborates with Reach Out and Read KC (ROR) to provide new books for children ages birth to five.
The founders of Reach Out and Read recognized the need to educate parents well before their children reached school-age. Because parents seek advice from pediatricians, providing books during well-child visits became the “vehicle” for reaching families. The new books provided by ROR are in English, Spanish, and many other languages. ROR also works with Hooked on Books to collect and provide thousands of gently used books in the waiting rooms for siblings of children seeing the provider.
